Roughly 3 percent of patients who undergo total hip and knee replacement surgery require critical care services before they are discharged from the hospital, according to an analysis of roughly half a million patients. The study, published online in advance of print in the July issue of the journal Anesthesiology, demonstrates that these elective surgeries are placing an increasing burden on the critical care services of the health care system and hospitals should respond proactively.
"This number is large enough that critical care physicians need to be very familiar with this patient population," said Stavros Memtsoudis, M.D., Ph.D., director of Critical Care Services at Hospital for Special Surgery in New York City, who led the study. "Hospitals need to understand that it is going to be a significant part of their patient population and allocate resources accordingly." He pointed out that the study also highlights the risks of these surgeries in older populations with comorbid conditions.
The number of hip and knee replacements performed in the United States yearly is currently over one million and is expected to surpass 4 million by the year 2030. While clinicians have noted that patients undergoing these surgeries have increasing comorbidities, until now, little research has been undertaken to see how these surgeries impact the use of critical care services (CCS).
To find out, researchers turned to a large inpatient database in the United States called Premier Perspective that is often used to study health outcomes. They analyzed data from all patients in this database who underwent hip or knee replacement surgery between 2006 and 2010. In 528,495 patients who underwent these surgeries at 400 hospitals, 3% required CCS. Patients who required CCS had higher mortality rates (2.5% vs. 0.1%), longer hospitals stays, higher costs, and were less likely to be discharged home (40% vs. 63%).
The investigators then compared characteristics of patients who used critical care services with those who did not. They evaluated demographics, comorbidities, hospital specific variables, and procedure specific variables, such as the type of anesthesia used.
The researchers found that patients were more likely to require CCS if they were older, the average age of 69 versus 66. Patients who needed critical care services also had more comorbidities. For example, the odds for requiring CCS increased by roughly 72% if patients were obese, roughly 106% if patients had advanced diabetes, and roughly 56% if patients had chronic obstructive pulmonary disease.
The researchers also found that the use of general anesthesia rather than regional anesthesia increased the use of CCS. This finding did not surprise Dr. Memtsoudis who says that HSS recognized the benefits of regional or neuraxial anesthesia for these procedures years ago. "Approximately ninety percent of the hip and knee orthopedic procedures being performed at HSS are done under neuraxial anesthesia, meaning spinal or epidural," he said.
The investigators found that the most common complications leading to use of critical care services were cardiac complications, including heart attack, and pulmonary compromise. Clinically, pulmonary compromise can range from shortness of breath to acute respiratory distress syndrome (ARDS), the latter of which involves prolonged time on a ventilator and is associated with a high risk of death. While any surgery is stressful on the heart, especially in older patients, joint replacement surgery carries a special risk for pulmonary compromise because of the very nature of implanting and cementing a prostheses.
"When a prosthesis is implanted, bone marrow, fat debris and cement particles can enter the blood stream of the patient," said Dr. Memtsoudis. "The first organ to see that load of embolic material is the lung. In this case, the lung serves as a filter with usually little clinically significant consequences. But sometimes, when either the load of debris is large or the lung itself is compromised by preexisting disease, the resulting inflammation in the lung can cause pulmonary compromise."
The new study provides data that will help clinicians assess the risk for complications in patients for hip and knee replacements. It also underscores the need for hospitals and critical care units to prepare themselves for the growing number of orthopedic patients who will need critical care services. "Orthopedic patients are not the prime type of patient that people expect in an ICU. It is elective surgery after all," said Dr. Memtsoudis. "Risk factors for needing critical care services are advanced age and existing coronary artery disease, diabetes, obesity and a number of other comorbidities. Unfortunately, this is the type of population that usually requires hip and knee replacements."
The researchers recommend that hospitals increase surveillance of high-risk patients undergoing hip and knee replacement surgery, a strategy that has been implemented at HSS. Two years ago, HSS established a team of six critical care physicians to attend to the critical care needs of their orthopedic patients. Now, high-risk patients undergoing joint replacement surgery receive more specialized levels of care.
"We identify patients who are at high risk for complications and we monitor those patients in our recovery room for at least one night. They stay on invasive monitors. We keep an eye on vital signs, pay attention to medication requirements, optimize interventions to address patients' medical problems and check appropriate labs frequently," said Dr. Memtsoudis. "If you have someone in an environment where you can observe patients closely, you can pick up abnormalities very quickly and intervene, thus hopefully avoiding more serious consequences."

George Zimmerman, charged in the murder of teenager Trayvon Martin, returned to jail Sunday two days after a judge revoked his bail. NBC's Charles Hadlock reports.

Updated 4 p.m. ET: George Zimmerman, the man charged with second-degree murder in the shooting death of Florida teenager Trayvon Martin, was booked into jail Sunday, two days after a judge revoked his bail after evidence surfaced that Zimmerman and his wife may have misled the court about their finances.

Zimmerman, 28, arrived in the state Saturday night from an undisclosed "secure location," where he has been staying because of "significant threats against his life," his lawyer, Mark O'Mara, said Sunday. He arrived at the jail before 2 p.m. on Sunday.
Zimmerman met police at a business park and was driven to the jail. He was handcuffed and wore a blue checkered button-up shirt. He is being held without bail and is listed as having $500 in his jail account, according to the jail website.?
Zimmerman, a neighborhood watch captain in Sanford, Fla., was released after posting $15,000 in cash on April 20 after he was charged with killing Martin, 17, who was unarmed, in February. The case triggered a nationwide debate about whether race was involved in the shooting; Martin was black, while Zimmerman is the son of a white man and a Peruvian woman.
Zimmerman was quiet and his return to jail went smoothly, Seminole County Sheriff Donald Eslinger said at a news conference Sunday. He said Zimmerman will stay in a single cell as he did before he was released in late April. The cell is 67-square-feet and is equipped with a toilet, two beds, a mattress, pillow, blanket and sheets, according to The Associated Press.

Seminole County Circuit Judge Kenneth Lester revoked Zimmerman's bond Friday after learning that Zimmerman and his wife, Shellie Zimmerman, may have misled him about their assets when he set bond. Prosecutors demonstrated that Zimmerman had at least $135,000 that he did not disclose in a special PayPal account he had set up to pay for his defense.
Zimmerman and his wife testified at a bond hearing that they had little money. During the hearing, Zimmerman also apologized to Martin's parents. The judge set bail at $150,000 -- days later, Zimmerman was released after putting forth 10 percent of the total bail amount.??
But prosecutors listened to recorded jailhouse phone calls and said the Zimmermans talked about the money "in code to hide what they were doing."

By concealing the defense contributions, Zimmerman benefited from a lower bond than he might have been granted had he and his family told the truth about how much money they really had, Lester said.
O'Mara said in a posting on the defense team's website that he would seek a new bond hearing, at which he would argue that "the vast majority" of the donated money was in an independently managed trust and that neither "Mr. Zimmerman or his attorneys have direct access to the money."
Wearing a casual, striped button-up shirt Sunday, O?Mara said that Zimmerman is concerned about his and his wife?s safety because of the ?enormous anger and frustration and hatred that has spurred from this case.?

Benjamin Crump, an attorney for Martin's family, said Judge Lester's finding that Zimmerman was dishonest is "very important because his credibility is the most important thing in this entire case." Legal experts also told The Associated Press that the judge's questioning on Friday could undermine Zimmerman's credibility in trial. ?
On the defense team's website, O'Mara wrote: "The defense team hopes that Mr. Zimmerman's voluntary surrender to Sanford police will help demonstrate to the court that he is not a flight risk."
But at Sunday's news conference, O'Mara added: ?As Mr. Crump said, there?s a credibility issue that needs to be rehabilitated by explaining away why they did what they did, if that?s what happened.?

We had a big party planned to celebrate Ben's 3rd birthday today.? We were going to have hamburgers, hot dogs, cake and ice cream.? We bought all the fixings and were ready to go, and then around midnight Will came down with the stomach bug.? LONDON/PARIS (Reuters) - Iran is poised to offer the Syrian authorities a short-term food lifeline with vital grains purchases as Western sanctions and mounting violence deter trade houses from doing deals with Damascus, international traders say.
Both are targets of Western sanctions that, while not intended to disrupt food imports, have hurt shipments of all kinds by complicating financial transactions. Richer and more practiced in the ways of sidestepping such embargoes, Iran seems set to help its struggling ally, though its own means are limited.
"Iran will try to help Syria," said a senior trader at a major international grain house in France who likened Tehran's interest in helping Syrian President Bashar al-Assad stave off food shortages to Algerian state aid last year for Tunisian and Libyan autocrats who were trying to stifle popular unrest.
"I think most of it will be done via the black market," the trader added, meaning that bread wheat, animal feed and other grains could be shipped to Syria by Iran discreetly, avoiding the normal practice of international public tenders.
Numerous sources in European grain trading centers and the Middle East concurred that Iran, a major energy exporter which has been under Western sanctions for years due to a dispute over its nuclear program, would help Assad, a key Arab ally whose government has been hit with Western and Arab League bans on banking and other dealings due to its crackdown on protests.
"It's a given that Iran will help Syria," said another grains dealer in western Europe, speaking on condition of anonymity, as is common in the business. "But it won't be on the radar. It will be a bilateral agreement between them."
Grains traders said Iran, which buys its supplies through public tenders and in private deals, would also aim to help Damascus by turning a blind eye to its own private merchants re-selling wheat to Syria.
Government agencies usually buy through public tenders where the quantity required, shipment schedule and other details are sent to international trade houses with a deadline for bids. Purchases by the private sector are generally less visible.
Separated by a land border, some of the ways Iran could get supplies to Syria would be on trucks via Iraq or Turkey, which have borders with Syria. Separately, shipments could be re-routed through Iraqi or Turkish ports and then onto Syria, European and Middle East grain traders say.
Unable to finance the big international grain purchases it has been used to, Syria has engaged in a desperate search for grain that has forced Damascus into an array of unusually small deals, many arranged by middlemen around the Middle East and Asia.
But the amounts agreed have been nowhere near meeting Syria's reliance on imports for about half of its annual needs of 7-8 million metric tons (7.7-8.8 million tons) of grain, a situation that threatens to sap domestic support for Assad as he faces mounting international condemnation and domestic defiance of his rule.
Tehran, under pressure itself for refusing demands from world powers to stop enriching uranium they see as part of a secret nuclear weapons program, has experience that can help an ally in the regional confrontation which pits Shi'ite Muslim, non-Arab Iran against the Sunni-dominated Arab states.
"Iran's relative success in circumventing sanctions, which have complicated efforts to import grains and other basic food staples, derives from its ability to find alternative ways to pay for imports," said Torbjorn Soltvedt, senior analyst at risk analysis firm Maplecroft in London.
Iran, unlike Syria, is increasingly working around the restrictions using covert payment systems including non-dollar currencies and its oil and gold in exchange for goods.
However, even Iran, needing to feed its 78 million people, has limited resources to help Assad, whose Alawite minority, religiously tied to Shi'ite Islam, has dominated Syria for four decades, even though most of its 23 million people are Sunnis.
"Although Tehran is likely to continue to provide financial and logistical support to Damascus, domestic food security concerns may limit the degree to which it will be able to meet a significant shortfall in the supply of grain and basic food staples in Syria," said Maplecroft's Soltvedt.
A trader based in the Middle East said Syria was most likely to benefit from profit-driven private traders in Iran rather than from direct aid from the Tehran government: "Iran has to continue to sort out its own food problems, so any assistance to Syria is not viable by the government," the trader said.
"It could become a private trade brokered by Iranian middle men," he added.
Traders involved in the supply of grains to the region say the coming harvests in Syria and Iran may also help the Syrians for a short period, though its food requirements remained critical.
"Syria's crop is coming in and this will take some of the immediate pressure off the government," said one trader in Hamburg, a key grain trading centre in Europe.
"The United Nations aid agencies are also doing big buying of Syrian aid cargoes, so the Syrian government may be happy to let the international aid agencies foot a large part of their food bill."
TURKISH INTERVENTION
Turkey was also likely to remain as an intermediary for both Iran and Syria, traders said. In recent days there have been grain deliveries to Syria via the Turkish port of Mersin, while Turkish banks have been used for Iranian and Syrian deals.
Turkey, exasperated by its one-time ally Assad, announced late last year its own sanctions targeting the Syrian government. But it specifically said it would avoid measures that would add to the hardships of the Syrian people.
"Turkey will remain the key factor for the Iranians and they still look like they are keeping lines open," one trader in western Europe said. "So bringing in supplies from Turkey is a viable option for Iran."
Syria has few options but to look for unconventional supply routes:
"I do not know anyone who is prepared to offer to the Syrians," another grain trader in France said. "For us it is an internal decision that is not only related to financing problems and risks of shipping but a way to put pressure to stop the atrocities in that country."
State currency reserves have been depleted and the Syrian pound has lost nearly half its value, adding to import problems.
The worsening trade environment has already hit Syrian container ship volumes, which account for household products such as canned food and clothing.
"There is a huge mess all over Syria today so how can you make sure your vessel will arrive and discharge safely with no problems, no demurrage (extra costs), let alone not being damaged by fighting?" the Middle East grain trader said.
"These are considerations facing most shipowners and will deter business now."

JERUSALEM (AP) ? For decades, Israel has marketed an array of cosmetics and food products manufactured in the occupied West Bank as "Made in Israel," blurring their true origins in Jewish settlements opposed by virtually the entire international community.
Now that practice is being challenged with demands that products made in the settlements be labeled accordingly.
Recent criticism, coming most prominently from South Africa, is putting Israel in a bind over the muddle it has created in the West Bank: Despite 45 years of control and a massive and costly effort to settle it with Jews, Israel has never annexed the territory ? and the Palestinians claim it for a future state.
The limbo yields some bewildering results, such as Jewish settlers casting ballots in home communities not in Israel proper despite there being no provision for absentee voting.
Asked to explain the "Made in Israel" label for products made outside Israeli territory, Foreign Ministry spokesman Yigal Palmor reached for an unorthodox defense, arguing that it is not intended as a geographical indication.
"My point is not the sovereignty over the West Bank, but who is the authority that has supervised the product," Palmor told The Associated Press. "Since the products of the settlements are made under Israeli regulations and standards, they are 'made in Israel.'"
He said there were other cases of contestable labeling, including by the Palestinians who label products from parts of the West Bank governed by the Palestinian Authority as "Made in Palestine." There is formally no state of Palestine, he noted.
The Palestinians have long called for a boycott of goods made in Israeli settlements and even have lit bonfires to destroy them.
"We are calling for boycotting the Israeli products simply because the Israelis shouldn't benefit from occupying our land and our people," said boycott activist Murad Sudani, who heads the Palestinian Writers Union.
Several weeks ago, South Africa issued a notice saying it wants to require merchants "not to incorrectly label products that originate from the Occupied Palestinian Territory as products of Israel." It said consumers should not be "misled" that the products originated in Israel.
The notice did not specify what the label should say, saying "the burden for proving where the products originate will lie with traders." The proposal has not taken effect, pending a 60-day period for public objections that can be submitted by the end of June. South Africa is not a major market for Israel.
Nonetheless, the voice of the South African government could be a symbolic boost to the Palestinians, given the country's history of overcoming apartheid and its leading role in the developing world. It would become the first country to require distinct labeling of settlement goods.
Others have begun to move in the same direction.
Danish authorities said this week they would soon introduce labeling guidelines for settlement products. Erik Jepsen, spokesman for the Danish Veterinary and Food Administration, said Thursday that the rules would be mandatory for fruit and vegetables and voluntary for other products.
Danish Foreign Minister Villy Soevndal said the government wants to raise awareness about the settlements, which the EU considers a major obstacle to Mideast peace.
The move follows a British decision in 2009 to allow retailers to distinguish whether West Bank goods are produced by Palestinians or Israeli settlers. It was not clear if there were cases in which this was done.
This week, Swiss supermarket chain Migros said it would tell its customers if products come from the settlements. The chain said it opposes calls to boycott Israeli products but wanted to offer customers greater transparency. Previously, it identified products as coming from Israel.
In perhaps the strongest international stance against the settlements, the European Union has excluded settlement goods from duty-free status given to other Israeli imports.
Yigal Dilmoni, a settler leader, said the criticism is unfair. "All the communities in Judea and Samaria are part of the state of Israel," he said, using the biblical terms for the West Bank. "So of course these products are made in Israel."
Settlement products account for less than 1 percent of Israel's exports of some $50 billion a year, according to the Manufacturers' Association of Israel.
Dead Sea cosmetics, high-end wines and dates are some of the products produced in the West Bank. Many of the businesses employ Palestinian laborers.
"We think that politics and economics shouldn't be mixed up. There is good economic cooperation with the Palestinians," said Dan Catarivas, director of the Manufacturers' Association's Division of Foreign Trade and International Relations.
Palmor said Israel is being unfairly singled out, claiming goods from other conflict zones don't face similar scrutiny. Morocco, for example, exports tomatoes from the disputed Western Sahara with a "Morocco" label.
The Palestinians seek all of the West Bank and neighboring east Jerusalem as parts of a future state. Israel captured the two areas in the 1967 Mideast war.
Although Israel has annexed east Jerusalem, the international community considers both territories to be occupied and opposes the presence of the 500,000 Jews living in the two areas.
Peace talks have been stalled for more than three years, with the Palestinians refusing to restart negotiations until Israel stops all construction in the settlements. Israel says peace talks should resume without any preconditions.

Dempsey deserves credit for everything he?s done for both club and country, but on Saturday against Scotland, Bradley showed he can drive a team in the absence of the USMNT?s biggest weapon. And even though Landon Donovan put in great performance in Jacksonville, he was off his game on Wednesday. Under Klinsmann (particularly during the strong run the team put in ahead of Brazil), the U.S. showed there?s some semblance of a life without Landon.
You can?t say the same about Bradley. In fact, it?s been the opposite, wit the U.S.?s recent success coming after Bradley settled in at Chievo. Club stability coincided with a return to the national team, when Bradley immediately reclaimed his spot in the midfield pecking order. It was only after the (still only) 24-year-old was integrated into Klinsmann?s plans that the U.S. started to resemble the hard working, pressing, possessing side the coach advertises.

Against Brazil, there wasn?t much pressing, and when the U.S. was possessing, it wasn?t for long. But Bradley?s quality still stood out. Along with Johnson, Bradley was one of the few States? players competing at the same level as their counterparts. His confidence on the ball was evident as he tested Brazil?s defense with 30-yard probes, trying to find Herculez Gomez running behind Juan and Thiago Silva.
And Bradley had more than the long ball in this?repertoire. On the U.S.?s only goal, he made the right read to get forward, play the ball to the line and utilize Johnson?s speed in the buildup to Gomez?s goal. It may have looked like an obvious play, but there are a lot of midfielders who could have unloaded (and probably put the ball into the stands). Given the goal Bradley scored against Scotland, he would have been forgiven for doing the same.
